We understand that smaller organizations may not be legally required to have all of these documents, but please be as complete as possible.

  • Most recent audited financial statement
  • If applying through a fiscal sponsor, upload proof of the sponsor agreement
  • Revenue Code or evidence that fiscal sponsor is tax-exempt under Section 501(c)3 of the Internal Revenue Code *required
